PROGRAM DESCRIPTION

The Aging Services Department is the focal point for a comprehensive system of services for older individuals in the Texoma Region.  These programs include:  Information and Referral, Caregiver Support, Care Coordination, Benefits Counseling (Legal Assistance), Long-term Care Ombudsman, and AmeriCorps Senior.

 

SUMMARY OF POSITION

The Community Health Worker will assess and address the needs of specific target populations as pertaining to their transition following discharge from an acute or post-acute setting. They will provide case management for a determined amount of time; evaluating appropriate resources and linking client and/or their family to services and providing support and education on chronic illness and healthy living with an overall goal of reducing the need for hospitalization. The Community Health Worker will also conduct group health and preventative screenings in our region.

 

ORGANIZATIONAL RELATIONSHIPS

  1. Reports to: Department Director
  2. Directs: May direct interns an/or volunteers
  3. Other: Collaborate with COG staff; federal, state and local agencies; and the general public

 

ESSENTIAL DUTIES

The essential duties of this position include but are not limited to the following performance measures:

  1. Establish trusting relationships with patients and their families while providing general support and encouragement.
  2. Serve as a liaison/intermediary between health/social services and community; facilitate access to services and improve quality of service delivery.
  3. Provide on-going follow-up and goal setting with patients/families and be able to identify risks for return to acute.
  4. Conduct face-to-face visits with patients before they are discharged from a facility, and then on a regular basis in the community during the determined service period.
  5. Increase health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.
  6. Maintain a timely record of clients served and documentation of services provided in state database.
  7. Conduct health and preventative screenings at off-site locations for groups of consumers.
  8. Establish and maintain a working relationship with service providers;
  9. Participate in short and long range strategic planning;
  10. Attend training, meetings and conferences related to program goals as determined necessary by supervisor;
  11. Perform other duties and responsibilities as required or assigned.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES

Must be able to maintain complete and accurate records; establish and maintain effective working relationships with local agencies and the general public; demonstrate proficiency in both written and oral communication; exhibit excellent computer skills including Word and Excel in a Microsoft Windows environment and data entry and maintenance of records in the State Database. Must be able to engage with clients in their own environment and be culturally competent.

 

EXPERIENCE AND TRAINING

Must have at least one year experience in a healthcare or social service organization. Preferred applicant will have health care education and background, or be pursuing a career in healthcare or social work.

 

CERTIFICATES AND LICENSES REQUIRED

Appropriate driver’s license with reliable transportation. Applicant should be a certified Community Health Worker or prepared to complete CHW certification within the first 90 days of employment. CPR and First Aid certified.
