This form is for new 911 addressing applications only.

TCOG does not assign 9-1-1 addresses to vacant lots or undeveloped properties.
Address numbers are assigned based on BOTH the driveway and structure location.
Every effort will be made to process your address request as quickly as possible. Due to the high volume of requests, it may take between 20-25 business days to complete.
You can help by responding to all the questions and providing all the requested information; in particular:
1. your Property ID Number,
2. the coordinates (latitude/longitude) of the structure's location, and
3. the coordinates (latitude/longitude) of where your driveway intersects with the nearest street.
If the nearest street to your driveway is maintained by the State of Texas, you will first need to obtain a culvert permit from TxDOT. Once obtained, notify us at maps@texoma.cog.tx.us so we can proceed with processing your application.
We will only process applications that are complete. Applications with missing, inaccurate, or incomplete information WILL NOT be processed.
